PT - home health needed asap Bachelors Degree or equivalent (some certifications will require Bachelor?s): Required Master's Degree: Preferred Physical Therapy licensure in state of practice: Required Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required Valid Driver?s License (DL) and must be at least 21 yrs of age or older: Required Physical Therapist (PT): Required Basic Life Support (BLS) or Basic Life Support (BLS) or RQIBLS or Heartsaver CPR (CPR): Required
Required Qualifications
Physical Therapist, Home Health Licenses: PT-CA Certifications: BLS(Copy needed) SSN Required DOB Required Work History: Should not be more than 30 days of gap in entire work history References: 1 Reference in entire work history Bachelors Degree or equivalent (some certifications will require Bachelor?s): Required
Valid Driver?s License (DL) and must be at least 21 yrs of age or older: Required
