Physical Therapist - Home Health - (PT - HH)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2300.88 - $2521.17
Required Qualifications
Active MA PT license at time of submission (client will not wait for license); BLS (AHA)
Minimum 1.5+ acute nursing care (hospital base) and Home Care experience. Private Homecare experience will not be considered. Certified home health experience including proficiency with OASIS required
COVID VACCINATION (dose date on or after 10/3/23) now required.
Exemption forms are available (Both forms REQUIRE Provider/Religious Leader Signatures), or please ask your delivery team for documents. IF APPROVED BY HEALTHSYSTEM TRAVELER WILL WEAR A MASK AT ALL TIMES WHILE ON SITE.
