Position Summary:
The Occupational Therapist (OT) evaluates, plans, and provides rehabilitative services to patients with cognitive and/or ADL physical functioning disorders. The OT develops individualized plans of care in alignment with regulatory and clinical practice standards, utilizing evidence-based practice to improve patient independence, safety, and quality of life.

Responsibilities:

• Screen and evaluate patients with cognitive/ADL functioning disorders and create individualized treatment plans.

• Deliver rehabilitative, skilled, and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.

• Monitor and document patient progress, adjusting treatments as needed.

• Evaluate outcomes and make referrals as appropriate.

• Provide consultation and counseling to patients, families, caregivers, and service providers.

• Collaborate effectively with supervisors and interdisciplinary team members.

• Prepare comprehensive discharge summaries and maintain accurate patient records.

• Document all required clinical and regulatory information; accurately track treatment minutes.

• Participate in team meetings, case conferences, utilization review, and required facility/company trainings.

• Supervise and support OT assistants, rehab aides, and/or students.

• Adhere to NBCOT/AOTA and state practice acts, codes of conduct, and professional ethics.

Submission Requirements:

• 1+ year of OT experience – Required

• New grads – OK (must have completed Level II experience; Level I observation alone not sufficient)

• LTC or Rehab experience – Required

• MO OT license – Required
