This is an excellent opportunity for a Registered Nurse who is passionate about community-based care and improving health outcomes for older adults and individuals with chronic conditions.

Community Health Nurse – Bronx, NY

Available Schedule
Tuesday-Saturday shift
Sunday-Thursday shift.

The Community Health Nurse will be making 4-5 visits per day. Home care experience is required.

What You’ll Do:

• Conduct skilled nursing visits and health assessments

• Deliver person-centered care using the full nursing process

• Educate members and families on health management

• Maintain documentation in accordance with regulations and policies

• Collaborate with the care team to support holistic patient needs

What We’re Looking For:

• NYS Registered Nurse license

• Prior experience with community health, home care, or PACE preferred

• Strong assessment, communication, and organizational skills

• Commitment to culturally sensitive and compassionate care
