Registered Nurse (RN) - Long-Term Care

Assignment DurationThis is a 13-week contract position.
ScheduleFull time Days (5x8s - 7am-3pm), every other weekend required, holidays as needed.
Job DescriptionThe Registered Nurse (RN) must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit. The RN will provide both direct and indirect skilled nursing care for assigned residents in a Long-Term Care (LTC) facility.
Responsibilities- Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to the Supervisor and other members of the Interdisciplinary Team.
- Contribute to the creation of a culturally relevant care plan that reflects nursing diagnoses and interventions to achieve desired resident outcomes.
- Recommending revisions to the plan of care after evaluating resident outcomes.
- Maintain current knowledge and competency in Nursing Practice, participating in the mentoring/orientation of staff.
- Provide leadership in the professional practice setting under the direction of the Nurse Manager.
- Current New York LPN licensure.
- BLS certification (AHA or RQI only).
- Minimum of 1 year of experience as an RN in a Long-Term Care facility.
