Required Licensure:

Required Experience:

Important Notes:

  • This is a hybrid role: Telehealth + in-field UAS assessments
  • Candidates cannot perform UAS assignments elsewhere
  • Will cover specific zip codes not the entire borough
Job Summary

Nurses conduct initial UAS assessments, evaluating health status, strengths, care needs, and preferences to guide individualized long-term care service plans. The role requires excellent organizational, communication, and clinical skills, with the ability to work independently in a fast-paced environment.

Job Responsibilities
  1. Conduct in-home and telehealth pre-evaluations of patients, including UAS assessments.
  2. Review consumer medical documentation and referral forms.
  3. Enter evaluation data into electronic forms and transmit as required.
  4. Document concerns, discrepancies, or issues identified during assessments.
  5. Support continuity of care, reducing fragmentation and duplication.
  6. Discuss healthcare options, supports, vendors, and waiver programs with consumers.
  7. Coordinate with the Call Center to enroll eligible consumers into managed care plans.
  8. Foster collaboration with interdisciplinary care teams to enhance service delivery.
  9. Collect quality review data and case notes in CRM systems.
  10. Promote health and illness prevention opportunities.
  11. Maintain knowledge of community resources, payor requirements, and services.
  12. Meet all standards outlined in performance criteria and bonus templates.
  13. Perform other duties as assigned by the Regional Nurse Manager or project leadership.
