Job Description

Connected Health Care is seeking an experienced Travel Physical Therapist (PT) to join our partner facility in Port Angeles, Washington. This opportunity offers the chance to make a real impact in a skilled nursing and rehab setting while enjoying the beauty of the Pacific Northwest.

As a Physical Therapist, you will evaluate, plan, and provide treatment programs to help patients regain function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. You will collaborate with an interdisciplinary care team to promote patient independence and support long-term recovery.

Responsibilities
  • Conduct physical therapy evaluations and develop individualized treatment plans.

  • Deliver evidence-based therapeutic exercises, manual therapy, and mobility training.

  • Document patient progress and update care plans as needed.

  • Educate patients, families, and staff on therapy techniques and safe mobility practices.

  • Collaborate with nursing, rehab, and medical staff to optimize patient outcomes.

Qualifications
  • Graduate of an accredited Physical Therapy program (DPT or MPT required).

  • Active Washington PT license (or compact eligibility).

  • Current BLS certification.

  • Minimum of 1 year of experience in a SNF or rehab setting preferred.

  • Strong communication and teamwork skills.
