Job Title: Case Manager

Short Description: The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation.

Complete Description:

We are seeking a dedicated and results-oriented Case Manager to join our team. This role is crucial in ensuring patient safety and optimizing care through effective management and resource utilization. The Case Manager will work collaboratively with interdisciplinary teams, patients, and families to navigate care plans and discharge processes.

Key Responsibilities:
  • Identify patient problems or situations affecting their treatment and assist families in adapting to healthcare needs.
  • Promote patient and employee safety with a focus on quality care and environment.
  • Coordinate with the interdisciplinary team to develop, manage, and direct clinical plans for safe and timely discharges.
  • Interact appropriately with patients based on their age and understanding level.
  • Refer patients and families to necessary outside agencies for non-healthcare related issues.
  • Document medical necessity and ensure compliance with all insurance certification requirements.
  • Maintain an up-to-date community resource system and facilitate access to services for patients and families.
  • Manage fiscal responsibilities and oversee billing processes to minimize financial risks.
  • Attend weekly meetings and document the plan of care and progress notes as required.
  • Participate in the continuous improvement of operations and team dynamics.
Required Qualifications:
  • Current licensure in a clinical discipline per state guidelines (RN, RT, or SW preferred).
  • Associate's Degree in a healthcare field (Bachelor's degree preferred).
  • Working knowledge of the insurance industry and government reimbursement.
  • Minimum of 2 years of experience in Case Management and Discharge Planning in an Acute Care or Rehab setting.
